Work Restriction
What the patient can and cannot do at work, and for how long. This is the record an employer acts on.
Each restriction notice is its own dated record
The selector at the top lists every work restriction notice issued for the patient by date and time. Choose one to view it, or start a new one.
Revise by issuing a new notice, not by editing the old one
Recovery rarely runs to the first estimate. Because each notice is kept, extending or easing a restriction is done by issuing a new one — which leaves what you originally certified, and when, intact. That matters if anyone later asks what the employer was told and on what date.

The restrictions
Work restrictions (click all that apply) — a checklist of activities, from bending, lifting, carrying and kneeling through climbing stairs, driving and getting in and out of a car, to household tasks (cooking, laundry, gardening), personal care (bathing, dressing, tying shoelaces), and office work (filing, data entry, copying).
Where a restriction needs quantifying, it can carry an amount and units — lift, 10, lbs — rather than standing as a bare activity name.
Structured limits are what employers can actually act on
"Light duty" means whatever the reader decides it means. "No lifting over 10 lbs" is unambiguous, checkable, and defensible — and it is the difference between a restriction that is followed and one that is interpreted away.
Typewritten work restrictions takes anything the checklist can't express.
Printing the notice
The notice can be printed directly from the screen — it is a document for the employer, not only a chart entry. Print the version you actually issued, and it stays on file as that dated record.

Keeping it current
A restriction with no end date never expires
Restrictions are dated for a reason — they are a clinical judgement about a period, not a permanent status. Review them at each re-evaluation and issue a revised notice, rather than leaving an old certificate standing.
